Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?

I also accept Self Pay.
Please Note: MANY INSURANCE COMPANIES DO PROVIDE OUT OF NETWORK benefits as well. If I am not in network with your insurance company, please inquire about those benefits.
Payment:
Cash, Check, MasterCard, VISA and HSA are accepted for payment. All payments, including copays or co-insurance are due in full at the time of the appointment (I am not able to carry balances for clients). A $25 fee will be added to checks that are returned NSF.
Cancellation Policy:
If you miss your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the $100 missed appointment fee. This fee will not be covered by your insurance carrier. All cancellations are to be made by calling my work number 847-737-5277 x108
